The UPMC Center for High-Value Health Care aims to advance access to comprehensive cardiac care by enhancing UPMC’s data and communications infrastructure to better identify and serve patients living with unmanaged hypertension, particularly those with unmet social needs. Program objectives include supporting primary care in implementing evidence-based hypertension management practices to improve blood pressure control and expanding care teams through effective integration of community health workers into primary care practices.
