Heart-to-Heart Lake County aims to reduce cardiac-related hospitalizations and mortality by fostering equity-centered, culturally grounded, and person-centered approaches that address the medical and social needs of people living with heart conditions. The initiative prioritizes populations disproportionately affected by heart disease and facing systemic barriers to care, including Medi-Cal eligible adults in Lake County who identify as American Indian/Alaska Native (AIAN) or Hispanic/Latino. Heart-to-Heart Lake County focuses on building trust with AIAN and Hispanic/Latino communities to inform more equitable and responsive care; training Community Health Workers/Representatives in culturally responsive cardiac care; expanding early screening and self-management tools; and strengthening referral pathways between primary and specialty cardiac care.
