At La Clínica del Pueblo, we know cardiovascular health is shaped by far more than what happens during a medical visit. Through our Collaborative for Equity in Cardiac Care (CECC) initiative, we are strengthening how we identify and manage cardiovascular disease for Latino, low-income, uninsured, and underinsured patients across Washington, D.C., and Prince George’s County. The project combines earlier risk screening, team-based primary and specialty care, remote patient monitoring, and stronger connections to food, legal, and social-support services to help patients stay healthier and avoid preventable complications. By integrating clinical care with community partnerships and real-time care coordination, CECC is designed to create a more responsive, equitable, and sustainable model for cardiovascular care.
