Lawndale Christian Health Center (LCHC) and researchers from the University of Chicago Medical Center (UCM) are partnering to reduce the risk of cardiovascular disease (CVD) among Black men with a history of incarceration, a population at significantly higher risk of CVD. LCHC and UCM aim to create a patient-centered, socio-culturally tailored cardiovascular risk reduction intervention that holistically addresses medical and social needs that incorporates clinical practice innovation, team-based care, community health workers with lived experience, and connections to community organizations such as the Urban Farm on Ogden, which sources fresh produce through VeggieRx and has an employment training program for justice-involved men.
