The Foundation for Black Women’s Wellness’ Social Delta Initiative is a community-clinic partnership designed to improve cardiovascular health outcomes in Dane County, Wisconsin, through person-centered, culturally responsive care and community-based support. Through its Reclaim Our Hearts program, the initiative combines heart health education, peer support, movement and wellness programming, and Community Health Worker support to help participants address both medical and social factors that influence cardiovascular health. The program was developed in response to persistent cardiovascular health disparities and barriers to sustained wellness, including chronic stress, social needs, health misinformation, and challenges navigating care systems. In partnership with Access Community Health Centers, the initiative seeks to advance equitable, community-informed approaches to long-term heart health and wellbeing.
