MedNorth Health Center aims to expand a Community Health Worker (CHW)-led, disease specific model of care by implementing culturally responsive, scalable cardiovascular disease interventions. With the goal of transforming primary care delivery for patients impacted by CVD and comorbidities such as diabetes, a team-based care approach will be utilized that strengthens communication across care teams, aligns clinic-based staff with community partners to address gaps in care, and more fully integrates CHWs into primary care systems.
