Gallup Community Health’s Collaborative for Equity in Cardiac Care program is focused on improving cardiovascular health equity for rural and underserved populations in northwest New Mexico through integrated primary care, care coordination, and Community Health Worker (CHW) services. The program helps patients with cardiovascular disease and related risk factors navigate barriers such as transportation, food insecurity, medication access, and access to specialty care. Through community partnerships, outreach, and culturally responsive care, GCH is working to improve chronic disease prevention, patient engagement, and long-term health outcomes. The initiative also supports the integration of CHWs into primary care to create more sustainable, community-centered healthcare services.
