Providence Health’s Northern California Heart Health for All Collaborative, in partnership with Hearts of Sonoma County, aims to address gaps in comprehensive cardiac care and enhance improve hypertension and cardiovascular disease risk management among underserved communities. Building on a longstanding cross-sector partnership of healthcare, public health, and community-based organizations, the initiative seeks to expand access to culturally competent, high-quality cardiovascular care for Black, Indigenous, and People of Color (BIPOC) populations across Northern California. Through community-based screening, multidisciplinary care teams, and strong referral pathways, Heart Health for All will connect individuals to tailored clinical and social services, including nutrition-focused programs. The initiative will also work to strengthen hypertension control and establish a sustainable model for addressing cardiovascular health and social drivers of health across Sonoma and then Humboldt counties.
