Capital Health’s Cardiac Opportunities for Resilience and Empowerment (CORE) program aims to improve health outcomes for people living with cardiovascular disease in medically underserved communities in Trenton, NJ. Utilizing cross-sector collaboration and community-based care solutions, including culturally responsive community health worker support and patient-centered self-management strategies, the program will reduce cardiovascular disease by streamlining referrals for tailored medical and social care. CORE’s health information exchange infrastructure will also support the development of a population health management dashboard to monitor utilization, identify gaps in care, and drive continuous improvement.
